Operative Treatment of Tillaux Fractures of the Ankle
Surgical Anatomy
An intra-articular tibial plafond fracture requires anatomic realignment.
Attachment of the anterior inferior tibiofibular ligament should be preserved on the fracture fragment (Fig. 2).
The superficial peroneal nerve should be preserved on exposure.
The physis of the distal tibia may need to be spared if still open.
Positioning
The patient is placed in supine position with a bolster beneath the ipsilateral buttock (Fig. 3).
The patient’s foot should be near the end of the bed to facilitate access and imaging.
A radiolucent table enables fluoroscopic imaging and confirmation of internal fixation and reduction.
