Case description


A 44-year-old man sustained a both-column acetabular fracture and 62-C7 body fracture following a motor vehicle accident.



Indication for MIPO


Operative treatment of the pelvis was indicated in this case with multiple injuries for postoperative care and rehabilitation. Because of the displaced posterior column and nondisplaced anterior column fractures, plating in the posterior column and long screw placement in the anterior column with MIPO can be performed.

a–c Preoperative AP and oblique projections of the pelvis and hips show a not-significantly displaced both-column fracture of the left acetabulum.


Preoperative planning


Following initial assessment the patient was deemed safe for operative fixation of his pelvic fractures on the second day after admission. After the decision was made that the case was suitable for MIPO, a thorough preoperative plan was created, including the surgical approach, the displacement of the fracture fragments, the reduction technique, and the appropriate implant ( Fig 15.6-2 ).

Preoperative plan. 1–2 Cannulated screws. 3–6 Cortex screws.
