Pelvis, acetabulum: a minimally displaced transverse fracture of the right acetabulum—62-B1
Case description
A 22-year-old man sustained a minimally displaced transverse, complicated pelvic fracture, and multiple lumbar transverse and spinous process fractures following a motor vehicle accident.
Indication for MIPO
Although the posterior column fracture is minimally displaced, the anterior column still has some displacement. Operative treatment was indicated in this case with multiple injuries with displaced intraarticular fracture. Anterior column plate fixation and percutaneous, posterior column long-screw placement are optimal and viable options, which can be performed with MIPO.
Preoperative planning
Following initial assessment, the patient was in good condition for operative fixation of his pelvic fractures on the second day after admission. After the decision was made that the case was suitable for MIPO, a thorough preoperative plan, including the displacement of the fracture fragments, the reduction technique, the surgical approach, and the selection of the appropriate implant was performed ( Fig 15.3-2 ).
Modified ilioinguinal approach
Close reduction of the anterior column
Insertion of the anterior plate
Insertion of the posterior long-lag screw
Operating room setup
Anesthesia
General endotracheal anesthesia was used.
