Case description
A 52-year-old man fell from a tree and sustained a minimally displaced left anterior column acetabular fracture and a comminuted distal left radial fracture. Only the acetabular fracture is discussed here.
Indication for MIPO
This is a minimally displaced anterior column fracture; operative treatment is indicated for rehabilitation reasons, such as pain relief, earlier range of motion, and earlier weight bearing. Percutaneous long-screw placement is a good option for this minimally displaced anterior column fracture and provides a secure fixation—it can also be readily performed.
a–d Preoperative AP and oblique x-rays and CT scans of the pelvis and hips show a comminuted left anterior column acetabular fracture through the roof extending into the left iliac wing. There are also minimally displaced fractures involving the left pubic rami and pubic symphysis.
Preoperative planning
A good preoperative plan, including the displacement of the fracture fragments, the reduction technique, the surgical approach and the appropriate implant, is performed (Fig 15.2-2).
