Chapter 22 De Quervain Release

Donald H. Lalonde, Alistair Phillips

ADVANTAGES OF WALANT VERSUS SEDATION AND TOURNIQUET IN DE QUERVAIN RELEASE

  • You can see active movement differentiating the abductor pollicis longus (APL) and the extensor pollicis brevis (EPB) so you can identify and release both compartments.

  • A De Quervain release is such a short and simple operation that eliminating the tourniquet and sedation, with their associated discomforts and inconveniences, is good for both you and the patient.

  • All of the general advantages listed in Chapter 2 apply to both the surgeon and the patient.

WHERE TO INJECT THE LOCAL ANESTHETIC FOR DE QUERVAIN RELEASE

Inject 10 ml of 1% lidocaine with 1:100,000 epinephrine (buffered with 10 ml lido/epi:1 ml of 8.4% sodium bicarbonate) subcutaneously in the center of the incision area.

  • See Chapter 1, Atlas, for more illustrations of the anatomy of diffusion of tumescent local anesthetic in the forearm, wrist, and hand.
