22.1 Shoulder


Range of Motion







Figure 22.1 Pendulum exercises: Have pt bend forward at the waist with the upper trunk supported by the unaffected arm resting on a low table or chair; rotate the affected shoulder in slowly enlarging circles to improve range of motion.







Figure 22.2 Wall climbs: Have pt stand at arms’ length from a wall w the affected shoulder closer to the wall; abduct the arm as far as possible; if abduction not full, then complete full abduction by climbing up the wall w the fingers; the pt may need to step closer to the wall as abducting. Repeat 5 × per session, 3 sessions per d.







Figure 22.3 Posterior capsular stretch: Adduct affected arm across chest. Use opposite arm to further adduct, stretching the posterior capsule of the affected arm. Hold for 10-15 sec; 5 × per session, 2-3 sessions per d.







Figure 22.4 Towel stretch: Place affected arm on sacrum. W a towel and the opposite arm further stretch the affected arm by pulling upward. Hold for 10-15 sec; 5 reps per session, 3 sessions per d.



Strengthening






Figure 22.5 Supraspinatus: Have pt abduct arm w thumb pointed down to 60°; use free-wts (3-5 lb). Hold for 5 sec; 12-15 reps per set and 3 sets per d.







Figure 22.6 External rotators: Pt lying on side externally rotates lifting a 3-5 lb wt to parallel and holds for 5 sec. Perform 12-15 reps per session and 3 sets/d.

Internal rotation: While lying on side as with external rotators, have pt place wt in lower arm and rotator toward the chest (internal rotation), lifting a 3-5 lb wt and hold for 5 sec; perform 12-15 reps per session; 3 sessions per d.







Figure 22.7 Scapular stabilizers: From prone position, w head on rolled towel and 3-5 lb wts in each arm, have pt pull scapulae together lifting the wt off the ground. Hold for 5 sec; perform 10-15 reps, 3 sessions per d.



22.2 Elbow


Stretching






Figure 22.8 Flexors: Have pt extend elbow and wrist and max suppination. Hold for 10-15 sec, 5 reps per session and 3 sessions per d.







Figure 22.9 Extensors: Have pt extend elbow and flex wrist w max pronation. Hold for 10-15 sec; 5 reps per session, 3 sessions per d.



Strengthening






Figure 22.10 Flexors: W elbow and forearm resting on thigh w 3-5 lb wt in hand, have pt flex the wrist and hold for 5 sec, then lower to a 3 count; perform 10-15 reps, 3 sets per d.







Figure 22.11 Extensors: W elbow and forearm resting on thigh w 3-5 lb wt in hand, have pt extend wrist and hold for 5 sec, then lower to a 3 count; perform 10-15 reps and 3 sets per day.







Figure 22.12 Rope exercises: Tie a 5 lb free-wt on a rope to an old broomstick. Hold arms out in front of the body and roll the broomstick elevating the wt to the stick then lower slowly. Start with 5 reps per session working up to 10-15 reps and 3 sessions per d.
