A New Interventional Field?

I applaud and thank Dr Peck for this excellent issue on ultrasound-guided procedures, which is a landmark effort bringing together gifted clinicians and researchers to share recent advances. The impact of ultrasound refinement for neuromuscular and sports medicine diagnostic imaging and procedural techniques cannot be overstated. This field has evolved through a combination of technological advances, clinician ingenuity, and market demand for less expensive point-of-care alternatives. The United States was slow to embrace MSK ultrasound in earlier years. Radiologists such as Dr Sofka and Dr Adler from the Hospital for Special Surgery and Dr Nazarian at Thomas Jefferson Hospital had great foresight and were a few of many that were gracious to open their doors to many of us for an introduction to this modality. Dr Jay Smith and Dr Jonathan Finnoff from Mayo Clinic were further instrumental in expanding this exposure among Physiatrists. The landscape of musculoskeletal and sports medicine use of ultrasound has been evolving rapidly, developing into what may be in the future an adjunct subspecialty with many interested fields (physical medicine and rehabilitation, sports medicine, radiology, rheumatology, and so forth). This reminds one of the impact interventional spine specialists made to spine care and interventional cardiology to cardiology. Not only does musculoskeletal interventional ultrasound provide alternative imaging and procedural avenues, but also the evolvement of biocellular medicine is sure to further challenge traditional treatment paradigms. This latter field has drawn considerable attention, is controversial to many, and will be further addressed in depth in the next issue of Physical Medicine and Rehabilitation Clinics of North America , the topic of which will be Regenerative Medicine. I hope and suspect that our colleagues will enjoy these two informative treatises.
